In conjunction with the HTC, the Colorado Chapter provides free medical alert identification jewelry to the bleeding disorder community in Colorado and surrounding states that the Colorado HTC services. Medical alert IDs can be instrumental in taking life-saving measures during an emergency.

ELIGIBILITY

Orders will be accepted for community members who are current patients at the Hemophilia and Thrombosis Treatment Center at the University of Colorado Anschutz Medical Campus only. Your patient status will be verified with the HTC prior to placing an order.

Orders are limited to one bracelet per patient per calendar year, or one necklace per patient per every two years.

ORDERING STEPS

1. Complete the Consent to Contact form HERE. We cannot process your order without this form.

2. Complete the Medical Alert ID order form below. Forms missing information will be rejected. After filling out the form, please save the form to your computer with the file name "YOUR NAME_Medical Alert ID Order".

Make sure you measure, and don't guess, at the wrist size for your bracelet. There are no reorders for bracelets that are the wrong size. We've included a link below for a printable tape measure if you don't have one.

3. Email your completed form to info@cohemo.org with the subject line "NEW MEDICAL ID REQUEST".

Order Confirmation and Delivery

Once your order is received please allow 5 business days for processing. During this time, your order form will be reviewed and your patient status will be verified with the HTC.

After verification your order will be placed and you will receive an email from the Chapter with an anticipated delivery date. Orders typically take 3-4 weeks for delivery.

In addition to wearing your medical alert ID as much as possible, we strongly encourage all patients, and their families, to enter your medical emergency information on your mobile device. Click on the instructions below for help adding emergency information to your device.
